Diese standardisierte Technik bietet zus\u00e4tzlich zu routinem\u00e4\u00dfigen ABI-Messungen&nbsp;neue Einblicke in die Fu\u00dfarterien und Mikroh\u00e4modynamik.<\/p>\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p><em>\u201cCritical limb ischemia with consecutive foot ulcers and minor amputations are below the measuring thresholds of ABI. Since peripheral interventions enter the region of the pedal arteries, additional criteria determining the improvement of macrocirculatory and microcirculatory hemodynamics are of utmost interest.\u201d<\/em><\/p>\n<p><cite>Tissue optical perfusion pressure: a simplified, more reliable, and faster assessment of pedal microcirculation in peripheral artery disease; Horstick, Messner, Grundmann, Yalcin, Weisser, Espinola-Klein; Vol. 319: H1208\u2013H1220, 2020, AJP Heart \u2013 American Physiological Society<\/cite><\/p><\/blockquote>\n<p>TOPP liefert Informationen \u00fcber die Pedal-Makrozirkulation und -Mikrozirkulation, die durch die ABI-Analyse allein nicht erreicht werden k\u00f6nnen.<\/p>\n<p>Mit TOPP sowie oszillometrischem ABI (TOPP-ABI) als All-in-one-Screening-Methode werden Patienten mit unbekannter oder unbest\u00e4tigter pAVK fr\u00fcher diagnostiziert und fr\u00fcher behandelt, um das Fortschreiten der Krankheit zu verlangsamen.<\/p>\n\n<h2 class=\"wp-block-heading\">Messprinzip<\/h2>\n<p>Nach dem Anlegen der Manschetten an Kn\u00f6cheln und Handgelenken sowie der optischen Sensoren an den Zehen \u00fcbt das System einen Druck von 180 mmHg auf alle Manschetten aus, der in Schritten von 10 mmHg verringert wird. Durch die Aufnahme der optischen Sensoren kann der Untersucher sofort feststellen, bei welchem Druckschritt die Zehen des Patienten die ersten Pulsationen zeigen. Dabei werden verschiedene Kennzahlen, wie der oszillometrische ABI, der PWI, die Amplitude oder die Anstiegszeit der Pulswelle gleichzeitig erfasst.<\/p>\n<p>Dar\u00fcber hinaus werden wichtige Indikatoren f\u00fcr die Vorfu\u00dfdurchblutung, wie z.B. die Zehenwellenformen, innerhalb derselben Messung erfasst.<\/p>\n<div class=\"wp-block-image\">\n<figure class=\"aligncenter size-large\"><img src=\"https:\/\/www.vascular-academy.com\/wp-content\/uploads\/2023\/11\/topp-pic-1024x334.png\" alt=\"\" class=\"wp-image-553\" \/><\/figure>\n<\/div>\n<p>Dar\u00fcber hinaus umfasst das TOPP-Messverfahren h\u00e4ufig einen Belastungstest zur weiteren Abkl\u00e4rung der Ergebnisse. Dies liegt daran, dass eine asymptomatische arterielle L\u00e4sion in den unteren Extremit\u00e4ten nach einer induzierten Belastung h\u00e4modynamisch relevant werden kann. Sie wird durch die Variation im Puls-zu-Puls-Intervall gemessen. Die ideale HRV variiert mit dem Alter und ist individuell f\u00fcr jeden Patienten.<\/td>\n<\/tr>\n<tr>\n<td><strong>PWG<\/strong><\/td>\n<td>Pulswellen-geschwindigkeit<\/td>\n<td>PWG ist die Geschwindigkeit, mit der sich der Blutdruckpuls durch das Kreislaufsystem ausbreitet und sollte im Allgemeinen \u2264 10 m\/s betragen. Es wird klinisch als Ma\u00df f\u00fcr die arterielle Steifheit verwendet.<\/td>\n<\/tr>\n<tr>\n<td><strong>ZEITDIFFERENZ<\/strong><\/td>\n<td>ms<\/td>\n<td>Die Zeitverschiebung, gemessen zwischen dem Startpunkt beider Pulskurven, zeigt den Laufzeitunterschied zwischen der linken und der rechten Extremit\u00e4t. Werte \u00fcber 50 Millisekunden gelten als abnormal.<\/td>\n<\/tr>\n<tr>\n<td><strong>AMPLITUDE<\/strong><\/td>\n<td>mmHg<\/td>\n<td>Die H\u00f6he vom Beginn des steilsten Anstiegs bis zum h\u00f6chsten Punkt der Pulskurve ist die Amplitude.<\/td>\n<\/tr>\n<tr>\n<td><strong>ANSTEIGSZEIT<\/strong><\/td>\n<td>ms<\/td>\n<td>Die Anstiegszeit markiert das Zeitintervall vom steilsten Anstieg bis zum h\u00f6chsten Punkt der Pulskurve. Sie sollte f\u00fcr normale Werte &lt; 200 Millisekunden betragen.<\/td>\n<\/tr>\n<tr>\n<td><strong>ANSTIEG ZU ABFALL<\/strong><\/td>\n<td>%<\/td>\n<td>Der in Prozent angegebene Quotient aus Anstiegszeit und Abfallzeit. Bei normalen Werten sollte er &lt; 33% betragen.<\/td>\n<\/tr>\n<tr>\n<td><strong>OSZILLO-METRISCHER INDEX<\/strong><\/td>\n<td>mmHg<\/td>\n<td>Der oszillometrische Index markiert die Druckstufe, bei der die h\u00f6chste Amplitude gemessen wurde und ist mit dem mittleren arteriellen Druck vergleichbar. Werte \u00fcber 120 mmHg gelten als abnormal.<\/td>\n<\/tr>\n<tr>\n<td><strong>TEMPERATUR<\/strong><\/td>\n<td>Hauttemperatur<\/td>\n<td>Hauttemperatur an den Zehen, die von den Sonden der optischen Sensoren erfasst wird. Seitenunterschiede von mehr als 3 \u00b0C sind zu beachten.<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<\/figure>\n\n<h2 class=\"wp-block-heading\">Medizinische Applikationen<\/h2>\n<ul
